Consumer Health Record Access & Complaint Information

Angels on the Horizon Therapeutic Center, LLC is committed to protecting client rights and maintaining transparency in accordance with Texas law.

Pursuant to Texas Health and Safety Code §181.105, clients are entitled to the following information:

Requesting Your Health Care Records

Clients may request access to or copies of their health care records by submitting a written request to Angels on the Horizon Therapeutic Center, LLC.

How to request records:

  • Submit a written request via email, secure client portal, or in person

  • Requests must include the client’s full name, date of birth, and a description of the records requested

  • Valid identification may be required prior to release

Records are released in accordance with applicable Texas and federal laws, including HIPAA, and within legally required timeframes.
